ATTENDANT OUTREACH

Helping People to Live Fully in Their Communities

Our Attendant Outreach services are available in Mississauga to adults with permanent physical disabilities who are 16 years old and meet eligibility requirements. The program is funded by the Ministry of Health and Long-Term Care and there is no cost to you if you are eligible for services.

Our goal is to provide you with attendant services that offer a greater range of housing and lifestyle choices, predictable services and consistent staffing and an opportunity for full and active participation in existing and future relationships.

Attendant services are provided by qualified Personal Support Workers and include non-medical personal support and essential homemaking services that are available on a pre-scheduled basis only, between 6:00 am and 12 midnight, seven days a week. We can come to your home, place of employment, or where you are attending classes in the pursuit of a degree, diploma or certificate.

Care coordination is provided by our team of Community Supervisors who conduct standardized assessments and, in partnership with you, develop individualized service plans which are updated when your needs change. (Types and amounts of assistance are limited by eligibility criteria and service maximums).

  • Preventative skin care
  • Changing non-sterile dressings
  • Dressing/undressing
  • Help with eating
  • Exercises/passive range of motion
  • Transferring positioning/turning
  • Escorts for shopping (limited availability)
Essential Homemaking or Instrumental Activities of Daily Living

  • Preparing meals/cooking/clean-up
  • Changing bedding
  • Light house cleaning
  • Laundry
  • Medication assistance (limited scope)
  • Garbage disposal
  • Pet care assistance (restrictions apply)
  • Caring for children (on behalf of and in the presence of Consumer)
  • Escorts to medical appointments (limited availability)
