Making it Easy to Connect With the Services You Need
Nucleus administers the Mississauga Halton LHIN Central Registry which provides one-stop access to community support services that meet the needs of vulnerable seniors, caregivers, and people with limited ability to obtain healthy food. Staffed by a passionate team of Nucleus staff, the Central Registry works on behalf of health system partners to connect people with the right services, assess eligibility, prioritize incoming referrals, and manage centralized wait lists.
Self-referrals as well as referrals from hospitals, community partners, health care professionals, individuals and caregivers in Oakville, Milton, Halton Hills, Mississauga and South Etobicoke are welcomed for the following services:
- Supports for Daily Living (in-home personal support and homemaking services)
- Caregiver ReCharge (in-home caregiver respite services)
- OSCR Short Stay Respite (out-of-home caregiver respite)
- Meals in Home Programs (food support programs)
