Helping Seniors to Live at Home as Long as Possible

For many frail seniors with complex health needs who face challenges with activities of daily living, there is still “no place like home.” Our Supports for Daily Living program helps eligible high needs seniors who want and are able to continue living in their own homes by 24-hour practical assistance with the essential activities of daily living that one cannot perform due to age related physical limitations or impairments. The Supports for Daily Living program is aimed at reducing unnecessary stays in hospital for seniors once their acute care phase is concluded and provides an alternative to Long Term Care facilities while enabling a senior to maintain their independence and integration in their communities.

Our services are provided by skilled Personal Support Workers (PSWs) for seniors 65 years or older who live in Mississauga, Oakville and South Etobicoke, require multiple visits each day and meet eligibility criteria. Services include pre-scheduled and urgent/on-call personal support as well as essential homemaking services, assistance with instrumental activities of daily living and safety/reassurance checks seven days a week, 24 hours/day, 365 days a year. Eligibility is determined based on a standard assessment and the level of services that you require. This program is funded by the Ministry of Health and Long-Term Care and there is no cost for seniors who meet the eligibility criteria.

Care coordination is provided by one of our community supervisors who is your primary point of contact and in partnership with you develops your service plan, conducts regular assessments, supports/guides you in how to access other community and health services that you may require and addresses any questions or issues you might have as they arise.

To make a referral, call the Central Registry at 905-281-4443, or download a printable referral form and email it to or fax it to 905-337-0770. We welcome self-referrals as well as referrals from hospitals, community partners, health care professionals, individuals and family members. Central Registry also coordinates access to the eight Supports for Daily Living program within the Mississauga Halton region.
