ATTENDANT OUTREACH
Attendant Outreach supports adults with permanent, physical disabilities to live with dignity wherever they are - whether it be at home, in their workplace or at school.
How it works
Our Attendant Outreach services are available to adults (16 years and older) living in Mississauga with permanent, physical disabilities who can direct their own care. The program is funded by the Ministry of Health and Long-Term Care and there is no cost to eligible individuals.
Our Providers
Our services are provided by trained personal support attendants.
What's included
Non-medical personal support and essential homemaking services offered between 6:00 a.m. to 12 midnight, 7 days a week.
We come to you
We come to wherever you are - whether it be your home, place of employment or volunteering, or where you are attending classes.
Care coordination
Assessment, service plans and supports are created in partnership with you.

How to apply
We welcome self-referrals as well as referrals from hospitals, community partners, health care professionals, individuals and family members.
