There's no place like home. Our Supports for Daily Living program helps seniors, with essential service needs, remain in their own home with assistance.
SUPPORTS FOR DAILY LIVING
Since 2008, our award-winning program has helped nearly 3,000 seniors avoid placement in long-term care by providing 24/7 access to non-medical support at home.
How it works
Our Supports for Daily Living program helps frail, high-needs seniors with assistance with activities of daily living, e.g. bathing, light meal prep, dressing, in their own home. The program is funded by the Ministry of Health and Long-Term Care and there is no cost to eligible clients.
Our providers
Our services are provided by skilled Care Coordinators and Personal Support Workers (PSWs).

Eligibility
High needs seniors (65 and older), who live in Mississauga, Oakville and South Etobicoke.

We welcome self-referrals as well as referrals from hospitals, community partners, health care professionals, individuals and/or family members.
